Eligibility

Age 18 or older and resides in King County Medicaid recipient Finds it difficult to travel due to medical or behavioral health challenges Wants to receive services (or the client’s legal representative is requesting services) Experiencing one or more behavioral health challenges, such as: anxiety, depression, bipolar disorder, post-traumatic stress disorder (PTSD), or other behavioral health disorder

Application process

Call and ask to speak with the IHMH Clinic Coordinator. Referrals are accepted from all sources, including self-referrals.

Fees

No fees. Medicaid covers 100% of the cost.

Interpreter services

Provides both in home and virtual behavioral health counseling and case management support services, as well as telepsychiatry consultations and medication management. Includes intake assessment and behavioral health diagnostics, with follow-up counseling and case management support by master’s level mental health counselors and social workers. Services are provided to those who reside in their own homes, adult family homes, assisted living facilities, and skilled nursing facilities or those who have difficulty with transportation/mobility to receive services in a clinic setting. Alcohol or substance abuse concerns cannot be the primary challenge.

EvergreenHealth's In-Home Mental Health (IHMH) team provides counseling and case management services to adult King County Medicaid clients virtually and at their private residence, adult family home, assisted living facility or skilled nursing facilities when situations exist that interfere with traveling to an agency setting for support services.
