Travels by van to support unhoused or unstably housed individuals by providing outreach, mental health assessments, medical services, and referrals. supports clients throughout the housing process, including until they are stable in their housing. Services include:
  • Preventive care
  • Connection to follow up care
  • Follow up post-discharge
  • Mental health and substance use assessments
