Eligibility

Serves residents in Ferry, Stevens, Pend Oreille, Lincoln, Spokane, and Adams counties, as well as members of the Kalispel Tribe of Indians, the Spokane Tribe of Indians, and the Colville Confederated Tribes who may benefit from support navigating health care, social services, or community resources. Referrals may originate from hospitals, health‑care providers, community organizations, statewide systems such as 211 or 988, or through self‑referral.

Call or submit a self-referral form online. A Community-Based Worker will then conduct an intake assessment.

Delivers coordinated care navigation and support services that connect individuals with Community‑Based Workers and trusted local organizations to address a wide range of health and social needs, including housing, food access, transportation, health‑insurance enrollment, behavioral‑health support, and other barriers to care. The Community Care Hub guides individuals through complex service systems, ensuring they receive timely assistance and ongoing follow‑up after hospital visits, provider referrals, or self‑referral, helping them stay connected to the resources necessary for long‑term stability and well‑being.

Serves residents in Adams, Ferry, Lincoln, Pend Oreille, Spokane, and Stevens counties by connecting them to a coordinated regional support network of approximately 130 Navigators operating across 30 community‑based organizations in Eastern Washington. The Network provides localized assistance, helps residents access services more efficiently, and strengthens collaboration among partner agencies to ensure consistent, community‑focused support.
