Medical Care offered at CHAS Health's Southgate Clinic

Offers a range of comprehensive medical services including primary care, pediatrics and women's health, specialty care and behavioral health care. Coordinates teams of providers to integrate care and create a patient centered medical home. Services for children include acute care, well-child exams, immunizations, and sports physicals. Women's health services include annual gynecological exams, pap smears, family planning, birth control, and menopause management. Some clinics offer specialty services such as HIV/AIDS, hepatitis C, and PrEP therapy as well as assisting patients with bone and joint conditions, nutrition problems, and diabetes or pre-diabetes. Select medical clinics can also offer dermatology and rheumatology care. Patients can request a referral for on-site behavioral health providers and psychiatric consultants who can start a short-term care plan or coordinate more intensive services with other community mental health providers.
