Assists individuals with securing essential health and social services for older adults, adults with disabilities and those who care for them, including - In-home services and support - Food and nutrition - Long term care options - Housing resources - Health and wellness - Medicare and Medicaid Helps eligible adults in applying for COPES, a state government program that pays for personal care and other services. Staff can meet clients in their homes or at specific community locations as well as assist via phone and email. Follows up with callers to make sure the referrals were successful and their needs are being met.
