Provides compassionate end-of-life care and support to patients and their families. Nurses meet with newly enrolled families in their homes to develop a plan of care for the person on hospice and provide resources to support their caregivers.
Services include:
- 24 hour "on call" for enrolled patient emergencies
- Skilled nursing with pain and symptom management
- Counseling, including spiritual, bereavement and nutrition,
- Physical therapy, occupational therapy and speech/language pathology
- Education on disease process, coping skills, care planning, and assistance for physicians with case management.
- Durable medical equipment and medical supplies.
- Caregiver respite
