Clinical case management to at-risk elderly living in their own homes. Attempts to prevent premature or unnecessary hospitalization or nursing home placement. Services include in-home assessment, psychiatric care, respite care, senior peer counseling, family caregiver support, and a 24-hour Information and Assistance line for elders.
