-Individuals with an HIV/AIDS diagnosis -Income at or below 300% Federal Poverty Level -Currently in need of additional mental health and/or substance abuse support -Support with sustaining mental health or substance abuse treatment -Currently housed, but struggling to maintain housing -Assistance with linkage to care
