Must be an Applecare recipient.
Age 16 and older.
Must meet one of the following criteria:
- Homeless or at imminent risk of homelessness.
- Living in a hospital, nursing facility, adult family home, or other institutional setting without stable housing options.
- Frequent emergency department or inpatient use because of unstable housing.
