Helps families and individuals connect to available resources. Resources available include housing, health insurance, child care, food assistance programs, and other kinds of services. The services available differ by area. Services are provided at eight regional hubs. Local care coordinators work with a person to help them apply for programs such as unemployment benefits, subsidized housing or child care, food assistance programs such as SNAP or WIC, Apple Health, and more.

Health Homes assists patients with chronic conditions by providing services such as case management, care coordination, health promotion, comprehensive transitional care and follow up, individual and family support, and referral to community and social services.

Provides financial assistance to Washington residents in need who meet eligibility requirements. Temporary Assistance for Needy Families (TANF/WorkFirst): -  Provides monthly cash grants and medical assistance to eligible low-income families with minor children in the home. -  Applicants attend a WorkFirst orientation and eligibility interview.   -  Pregnant and parenting minors may be eligible for TANF benefits if they meet additional eligibility requirements, as determined by a DSHS social worker.   -  Adults may also be eligible for WorkFirst support, which will pay for items clients must have in order to look for work, prepare for work or accept a better job. -  Assistance may include transportation, car repairs, interview and work clothes, licenses and tools and equipment.   -  Assistance will be provided via vouchers or pre-paid merchant cards.   Aged Blind and Disabled (ABD): -  Provides financial assistance for people with a long-term or permanent disability, or who are blind, or who are age 65 or older and meet income and resource requirements.     Emergency Assistance: -  TANF, Workfirst, SFA and RCA clients may apply for supplemental financial assistance when faced with an emergency caused by circumstances out of their control.   -  Funding may be approved for rent (EVICTION NOTICES); electric, gas, heating fuel, water/sewer or telephone utility shut-off notices; or deposit move-in costs for homeless individuals and families.   -  Includes Additional Requirements for Emergent Needs (AREN). AREN payments may be made up to a maximum of $750 in a 12-month period.   - Also includes WorkFirst support services, which helps with rent, mortgage, taxes, insurance, utilities, moving costs, all housing-related fees, hookup fees, refundable and non-refundable deposits, and temporary housing for households participating in WorkFirst. Up to $5,000 is available in a 12-month period. Pregnant Women's Assistance: - Provides financial assistance for pregnant women who are not eligible for TANF but meet TANF income and resource requirements. Diversion Cash Assistance (DCA): -  Provides a cash grant program designed to prevent households with children from having to enroll in public assistance, by assisting with living expenses for needs such as: - Housing - Transportation - Child care - Food and employment-related expenses.   Clients may be referred to DSHS non-cash programs such as food stamps. Staff may also refer to other social service programs including domestic violence services, clothing banks, food pantries, etc. Housing and Essential Needs (HEN): -  Provides housing and essential needs vouchers for adults who have a shorter-term disability and meet income and resource requirements.   Refugee Cash Assistance (RCA): -  Cash assistance is available to refugees with no minor children for the first eight months after their arrival.   -  Refugees with minor children are likely eligible for TANF.   State Family Assistance: -  Cash assistance program established for legal immigrants with dependent children who are ineligible for TANF because of immigrant status.

Provides a daily drop-in center for unhoused women and non-binary individuals; no children allowed. Offers the following services: -  Limited food to-go -  Breakfast, lunch and snack - Computer use - Recliner chairs for resting - Phone available for use - An address for receiving mail - Job search assistance - Resume writing support -  Laundry facilities - Shower facilities with wheelchair accessibility, grab bars and a shower chair - Emergency clothing -  Hygiene kits      Offers light case management services with information, referrals and advocacy for various needs.   Clients are welcome to stay at the day center all day or drop in to access specific services.   Food and snacks are available throughout the day, not only at mealtimes. Interpreter services are available by advance request.

Case managers help HIV/AIDS-affected individuals obtain health insurance, find care and related resources, and provide general advocacy and support.

Provides domestic violence advocacy services, such as: -  Case management -  Counseling -  Support groups -  Legal advocacy   Staff will accompany refugee women and their children of any ethnicity to shelters and will attempt to meet their needs by providing crisis intervention, support, translation, etc.  Also offers education and training to the community and service providers about domestic violence and refugee/immigration issues.   Child care is available on a limited basis.

Helps households on the Housing Choice Voucher (HCV) Section 8 program work toward financial independence by providing one-on-one coaching. Supports families in achieving goals related to employment, education, homeownership, health, and more.

Friends of Youth's Healthy Start program service provides home visiting for pregnant and/or parenting families with children younger than six. Home visitors provide a safe space and meet families while they are at while providing basic supplies, parenting support and information, group activities, development and health screenings, and referral to community resources. Program serves families across east King County.

Offers programs to support and educate pregnant women and mothers of children up to 2 years of age. Programs include: Bebes! HOPE (Healthy Outcomes, Prevention and Education): A center-based case management program offering support, referral and resources Bebes! BSK (Best Start for Kids): A home visiting program twice a month, resources, referral and support for pregnant women and parents with children up to two years of age ParentChild+ Program: A home visiting program with 30 minute visits twice a week, helps prepare children for school, supports parents and offers free books and toys to enhance learning for children 18-30 months; Growing and Learning Together (PAT): A twice a month home visiting program for pregnant women or with infants and toddlers, not attending a child development center. Includes educational workshops and at home visits for individualized help.

Offer pregnancy, post-pregnancy, breast/chestfeeding, and parenting services at public health centers across King County. Not all services offered at all locations. Maternity Support Services (Part of First Steps): Helps people have healthy pregnancies and recover from pregnancy. Coordinates support until the baby is two months old, and provides breast/chestfeeding support by: - Health education and counseling. - Regular visits in the clinic, by phone or video, or in the client’s home or community setting. - Support beyond the doctor or midwife, including nurses, nutritionists, social workers and community health workers. - Referrals to OB care, WIC, health insurance and other community resources. Infant Case Management (Part of First Steps): - Provides support and guidance from the time the baby is 2-3 months old through baby’s first birthday (for those who qualify). - Helps clients become self-sufficient in gaining access to medical, social, educational and other services they might need. Nurse Family Partnership: Public health nurses provide care coordination services through home or community visits or on the telephone - Partners pregnant people with registered nurses from pregnancy through their child’s 2nd birthday - Delivers the support first-time parents need to have a healthy pregnancy, a healthy birth, and a healthy infancy for the baby. Children and Youth with Special Health Care Needs: Public health nurses provide care coordination services through home or community visits or on the telephone. Services include: - Developmental screenings and assessments - Help with concerns such as feeding, nutrition, growth, development and behavior. - Help connect CYSHCN families to the health and related services they need to thrive. Family Ways: - Provides culturally relevant peer support for three community groups: Native American/Alaska Natives, Pacific Islander/Native Hawaiian, and U.S. born Black/African Americans from pregnancy through age 5. - Supports clients with pregnancy, parenting, community-connections and resource navigation - Registered Dietitian, Social Worker, and Public Health Nurse available for consultation and coaching

Offers a wide range of services to meet the diverse needs of Spokane County’s youth. Case managers work alongside participants to address: - Supportive Housing: Housing searches, move-in cost assistance, subsidized housing referrals, and Spokane’s Coordinated Entry system. - Supportive Employment: Resume building, job applications, job searching, and employment sustainability. - Education Support: Connections to colleges, scholarships, GED programs, and trade schools - Family Reunification: Facilitated family meetings to heal relationships and strengthen bonds - Mental Healthcare: Individual therapy, coping skills development, motivational interviewing, and emotional support. - Resource Navigation: Assistance with EBT, SSI, vital documents, emancipation guidance, and referrals to other agencies. - Life Skills Development: Practical skills like cooking, budgeting, shopping, and relational coaching.

Offers individual job search assistance and job preparedness workshops for youth, adults, dislocated worker, migrant seasonal farmworkers, and veterans. A computer laboratory and resource room are available for job searching and career planning activities. Provides: -  Training programs that include Youth, Adult, Dislocated workers - Job Corps (GED/High School program, vocational skills training, and social skills training for ages 16 -24) - Job Search workshops for, resume, interviewing and applications - WorkFirst (assists parents receiving TANF to gain or improve work place skills to enhance employability and to obtain unsubsidized employment) -  Case management -  Job readiness training -  Career counseling -  Vocational assessment -  Individualized job search assistance -  Job fairs -  Employer connections -  Career development workshops -  Follow-up services -  Computer labs  -  Resource library 

Offers intensive mental health care and team-based case management for youth with complex needs and their families. Looks for community-based options to support multi-system-involved children and youth. Staff members trained in team facilitation meet with a family, listen to their story, help the youth build a team of natural and formal supports, and guide this team through the wraparound process. Team members include natural supports (such as family, friends, and religious leaders) and the professionals who work with the family (such as counselors, schools, Child Protective Services and probation officers).

Offer supportive services to unpaid adult caregivers. Caregiver support services may include:   -  Individualized comprehensive needs assessments and develop care plans -  In-home or office visits within King County -  Referrals to support groups, counseling and other resources -  Advice on the use of supplies and equipment -  Limited respite for the caregiver

A shelter for single women and women with children. Call to be screened onto the waitlist. The waitlist may be several months long. Does not offer financial assistance, legal assistance, or counseling.

Statewide program provides community-based outreach, intake, assessment, and connection to services and, as appropriate, long-term intensive case management and recovery coaching services. Recovery Navigator staff facilitate and coordinate connections to a broad range of community resources for youth and adults, including treatment and recovery support services.

Provides a network of care providers who offer comprehensive, up-to-date resources to assist Sickle Cell carriers and their families with: - Access to Washington state's most dedicated sickle cell specialists - physicians, nurses, social workers, genetic counselors and more. - Opportunities to learn about new ways to prevent and manage pain, to work with teachers and schools, and to otherwise support a child's health, growth and development. - Social activities and the chance to meet and learn from other families who are living with sickle cell disease. - Regular updates on the latest research, community activities and other resources. Provides assistance in patient-provider communication issues.

Provides civil legal advocacy to youth primarily between 12-24 years old. TeamChild’s legal team works to secure community-based services, help youth access appropriate education, healthcare, mental/behavioral health resources, safe and stable housing, navigate school discipline, and works with youth incarcerated in the Juvenile Rehabilitation (JR) system. Advocacy efforts focus on education and school-related issues such as enrollment, school discipline and access to special education; securing safe and stable housing for youth; and helping clients access public benefits, including health care, mental health services, DDA and other support. Advocacy also includes post-conviction relief, including records sealing, restitution modifications, and assisting youth with reentry supports when returning to community from incarceration. Note: TeamChild does not provide criminal defense representation to youth or adults involved in the juvenile delinquency or adult criminal systems, nor does it offer family law representation, including child custody issues.

Provides support and services for patients and families living with a diagnosis of Parkinson's disease. Services include support groups, exercise and speech classes, education workshops, case management, and counseling. Offers hybrid formats, online, in-person groups, and outdoor activities.

Provides rapid re-housing services, including outreach and short-term case management, to Veteran households experiencing homelessness in King County. Temporary Financial Assistance may be provided, depending on eligibility. Case managers are able to meet in the community.

Supports individuals who have serious chronic conditions and more than one medical or social service need. Care coordinators meet with individuals to assist in developing a health action plan. The care coordinators stay in touch with individuals and the agencies that support them to keep things moving forward. If individuals go in and out of the hospital, the care coordinators assist in planning transitions.
